Removes Markdown link destinations and inline formatting before slugging.

A run of underscores between two alphanumeric characters (foo_bar, foo__bar) is literal text, as in Python-Markdown, whose underscore emphasis never opens or closes inside a word; the toc slug keeps it (N94).
